Why Do Period Cramps Hurt So Much? The Science Behind Menstrual Pain

Menstruation commonly known as periods or the monthly cycle is not an illness. It is one of the most natural biological processes in a woman’s body. The English word menstruation originates from the Latin word mensis, meaning “month.” Ancient civilizations noticed that, much like the moon completes its cycle in approximately 28 days, a woman’s menstrual cycle also follows a monthly rhythm. This association led people to connect menstruation with the cycles of both the moon and nature. In Hindi, it is called Masik Dharma or Ritusrava, where Masik refers to something that occurs every month and Ritu symbolizes nature’s recurring seasons and cycles. From a scientific perspective, every month the female body prepares itself for a possible pregnancy. When conception does not occur, the thickened lining of the uterus is shed through menstrual bleeding. This process is so fundamental that human life itself could not continue without it. For centuries, however, society has held deeply contradictory views about menstruation. History and culture reveal a fascinating paradox. Many ancient civilizations regarded menstruation as a symbol of purification, fertility, and the regenerative power of nature. Even today, India’s famous Ambubachi Mela at the Kamakhya Temple celebrates the menstruation of the goddess as a sacred event. Yet ironically, the very society that worships a menstruating goddess often remains silent about the periods and pain experienced by its own daughters. In earlier times, when sanitary products and hygienic facilities were unavailable, keeping menstruating women away from kitchens or household chores may have originated as a way to provide them with much-needed rest from physically demanding work. Over time, however, this practice transformed into restrictive customs, superstition, and stigma. Even in today’s modern world where women excel in every field imaginable menstruation continues to be treated as an uncomfortable subject that people hesitate to discuss openly. This silence begins in childhood through subtle psychological conditioning. When a mother tells her daughter to hide sanitary pads inside black plastic bags or wrap them in newspaper before bringing them home, she unknowingly teaches her that menstruation is something shameful that must remain hidden. This culture of concealment gradually evolves into a dangerous habit of hiding pain itself. Girls grow up believing that menstruation should never be talked about, and as a result, many silently endure severe and debilitating pain, accepting it as an unavoidable part of being a woman. Society has normalized this suffering to such an extent that unbearable menstrual pain is often dismissed as “just a part of womanhood.” Treating this pain as normal has consequences that deserve careful examination. The only apparent benefit of this mindset is the convenience it offers to society and families. Women continue managing households, workplaces, and responsibilities while swallowing painkillers and suppressing their suffering, ensuring that daily routines remain uninterrupted. But this superficial convenience comes at an enormous cost. One of the harsh realities of modern medicine is the long-standing gender bias in medical research. In her acclaimed book Invisible Women, Caroline Criado Perez demonstrates, through extensive evidence and data, how medical studies, drug trials, and healthcare systems have historically treated the male body as the default standard. Conditions such as erectile dysfunction in men have received significantly greater research funding than debilitating menstrual disorders affecting millions of women. This imbalance helps explain why conditions like endometriosis still take an average of seven to eight years to be accurately diagnosed, allowing the disease to progressively damage women’s health. Gloria Steinem once made a striking observation that captures this inequality perfectly: if men menstruated, periods would be celebrated as a symbol of strength and masculinity, and governments would compete to invest the largest budgets in research and treatment. When severe menstrual pain is routinely ignored, serious conditions such as endometriosis, polycystic ovary syndrome (PCOS), and uterine fibroids often continue progressing without timely diagnosis or treatment. Delayed medical care can eventually lead to infertility, chronic pelvic pain, and lifelong health complications. Beyond the physical consequences, enduring intense pain in silence month after month takes a profound psychological toll. Many women develop anxiety, emotional distress, and depression while feeling isolated in their suffering. Global data reinforces the seriousness of this issue. According to the World Health Organization (WHO), approximately one in ten women worldwide lives with endometriosis. In India, various studies suggest that nearly 70–80 percent of girls and women experience severe menstrual cramps every month, yet only a small fraction ever consult a gynecologist or seek professional medical care. The impact is especially significant among today’s younger generation. Young women are pursuing higher education, competing in sports, and building successful careers alongside men. Yet every month, severe menstrual pain and persistent social stigma create major educational, economic, and professional barriers. Menstrual pain contributes to absenteeism from schools, colleges, and workplaces, resulting not only in personal setbacks but also in broader economic losses through reduced productivity. Many working women avoid discussing their symptoms out of fear that they may be perceived as weak or less capable. This has also fueled an ongoing debate around menstrual leave. While many view paid menstrual leave as a basic workplace right that recognizes women’s health needs, others worry that mandatory leave policies could unintentionally discourage employers from hiring women. It is a complex issue that demands thoughtful, evidence-based, and empathetic discussion rather than polarized opinions. Lifestyle changes have also intensified the problem. Previous generations generally consumed simpler diets, engaged in greater physical activity, and experienced comparatively lower levels of chronic stress. Modern life, however, is marked by processed foods, prolonged hours in front of computer screens, inadequate sleep, sedentary routines, and relentless work pressure. Together, these factors contribute to hormonal imbalances that may worsen menstrual symptoms for many women. At the same time, today’s generation has access to healthier and more sustainable menstrual products, including menstrual cups, period underwear, and digital period-tracking applications. These innovations represent important progress for both personal health and environmental sustainability. However, technological advancements alone cannot eliminate menstrual stigma. While conversations about periods are becoming more open on social media, meaningful change is still slow in many rural communities and small towns, where silence and misinformation continue to dominate. Ultimately, it is time to stop calling debilitating period pain “normal.” This is not merely a personal issue affecting individual women it is a broader public health, social justice, and human dignity issue. We must move beyond the outdated stereotype of the “ideal Indian woman” who silently endures pain as a measure of her strength and virtue. Endurance is not admirable when it comes at the cost of one’s health and well-being. Real change will begin only when families recognize severe menstrual pain as a legitimate medical concern, when men become informed and empathetic allies in these conversations, and when healthcare systems treat women’s pain with the seriousness it deserves. Until then, half of our population will continue to be prevented from reaching its full potential. The first step toward that change is remarkably simple: menstruation is not a source of shame, nor is it an illness. It is the very process through which life begins. And freedom from unbearable menstrual pain is not a privilege it is a fundamental right that every woman deserves.
